ELIGIBILITY
This application will be evaluated by an Award Committee. The application will be evaluated based on the responses in each of the following sections:
- Section I - Leadership
- Section II - Education
- Section III - Continuing Education
- Section IV - Awards
- Section V - Participation in Community or Social Service Activities
- Section VI - Liaison or Participation with Other Allied Health Organizations
- Section VII - Political Involvement and Legislative Activity
- Section VIII - Presentations and Authorships
- Section IX - Essay
Each section will be evaluated independently to achieve a total composite value for each nominee evaluation. The total composite score from all council members will determine the recipients. The Alfred C. Fones Award will be presented to the applicant receiving the highest score above the established minimum requirements. This award may not be given each year. In addition to the criteria above, the applicant must have been/is a member of the American Dental Hygienists' Association for at least 25* cumulative years. This needs to be verified by the Treasurer or Membership Chair of your Constituent by completing the Membership Verification Form.
*25 years of cumulative Active ADHA Membership is required for a member to be nominated for this award. This does NOT include Supporting or Student Membership
